New Student Registration
To enroll a new student in the Belvidere District #100 you may print off a New Student Registration Packet below or pick up from the Belvidere District #100 Central Office at 1201 5th Avenue in Belvidere.
Documentation required with completed registration packet includes:
- Original Birth Certificate
- Parent/Guardian Photo ID
- Legal Custody Documents
- Current Tax Statement or Current Lease and
- Two (2) Utility Bills: Gas, Electric, Water or Trash
- Phyical and Immunizations Records

2017-2018 Kindergarten Registration
2017-2018 Kindergarten Registration-Spanish
All new Kindergarten students regestering for the 2017-2018 school year must return completed documents to student's home school. Please bring your child to the school at that time to be screened.
For any high school students that are pre-registering for the 2017-2018 school year, use these forms. The district will start accepting 2017-2018 registration forms for all new students after June 1, 2017.
Address/Information Changes for Existing Students
All address/student information changes must take place at the Belvidere CUSD 100's Central Office.
Required Documentation for Address Changes:
Three current proof of residency documents which are addressed to the guardian and verifies the students address.

Current Lease-must be dated during the school year of registration. Lease must be signed by all parties including landlord’s name and phone number.

- Electric Bill
- Gas Bill
- Water Bill
- Refuse Bill
NOTE: All utilities must state the service address and the mailing address from the same location.
