Athletic Requirements to obtain an Athletic Permit

Each student athlete must have the following COMPLETED BEFORE an Athletic Permit will be issued allowing participation in any Belvidere School District athletics:

   *On-Line Athlete                 Registration

Each student athlete must complete the athletic registration process on-line at http://il.8to18.com/belviderehs. Through this process both parent and student athlete will acknowledge reading and accept the implications in the Code of Conduct,  IHSA Eligibility Rules, IHSA sports medicine Consent and aknowledgement, District100 drug testing policy and Procedure, Parent/Guardian & Athlete Release. Instructions for On-Line Athletic Registration are below.

  *Current Physical Examination

Each student athlete must have a current physical exam form or statement from his/her doctor indicating that he/she is physically fit to participate in sports.

This means no student will be allowed to try out or participate in any sport unless a physical exam is complete and turned into the Athletic Director’s office at the high school. This sports physical can reveal medical problems such as heart murmurs, high blood pressure, or other serious conditions that need to be treated before a student participates in a sport. Sports physicals are good for 13 months from the date the physical is completed.
**9th grade physical may be used as a sports physical. However, a sports physical may not be used as a 9th grade physical!

Physical form available on Forms Tab

   *Impact Concussion Testing

All student/athletes will be required to complete the impact concussion test prior to issuance of an athletic permit. The test is a computerized test that sets a baseline. Should an athlete receive a concussion, the athlete will need to exceed this baseline in order to be cleared to play.
Please note that ONLY incoming freshman, new students and students that have not already been tested need to attend a testing session.
Test sessions are assigned prior to the start of each sports session in conjunction with the parent/athlete meeting.
